Before you submit a request, check these resources to see if the information is already available:
Media Requests: If you are a member of the media, contact the Office of Communications at
[email protected] for assistance.
Individual Medical Records: If you need a medical record for a current or former Medicaid participant, contact their
HealthChoice health plan directly. For other medical records requests, see the
