Health Home Application Process
The following provider types are eligible to establish a Health Home program: Psychiatric Rehabilitation Programs (PRP), Mobile Treatment (MT) programs, and Opioid Treatment Programs (OTP). Providers interested in establishing a Health Home program must complete the Health Home Provider Application below and submit it to mdh.healthhomes@maryland.gov. Please refer to the additional documents below for guidance.

Prior to becoming a Health Home, providers must be enrolled in Maryland Medicaid. If you are not already enrolled in Medicaid as a PRP, MT, or OT​​P provider, for instructions on how to enroll through ePREP, see Provider Enrollment​.
