The Maryland Department of Health (MDH) invites local health departments and county governments to apply for participant spaces in the Assistance in Community Integration Services (ACIS) program.

MDH will award 1,520 spaces to new and existing providers and lead entities.

How to apply

  1. Request the mandatory letter of intent (LOI) template and instructions: Email [email protected].​
  2. Submit a LOI: Due March 13 by 5 p.m.
  3. Submit the Request for Responses/Application: Now Due April 6, by 5 p.m.

Become an ACIS Program Provider

​​The Assistance in Community Integration Services (ACIS) Program delivers housing and tenancy-based services to eligible individuals who are experiencing homelessness or at risk of becoming homeless. For more information about the program, see ACIS Program.

Who can be an ACIS provider?

​A local health department or other local government agency may apply to become an ACIS provider and deliver services within their jurisdiction. For more information, see the resources section.

Do I need to be an enrolled Maryland Medicaid provider?

You must be an enrolled Maryland Medicaid provider to deliver and bill for ACIS services. To learn more, see Provider Enrollment​.

How do I bill for services?

An ACIS provider must provide a minimum of three services per month for each member to receive reimbursement each month. For more billing information, see the ACIS Pilot Protocol.
