Step 1:
Click the button below to begin the appointment-booking process.
Step 2:
Complete the new patient registration form. Please review your entry for accuracy prior to submission.
Step 3:
Review and acknowledge the COVID-19 testing consent form.
Step 4:
Choose a location, date, and time for your COVID-19 test.
