Attending clinicians should:

• Complete the Attending Clinician Attestation Form portion of the Out of Institution Birth Form. 
• Complete the Mother's worksheet and the Facility worksheet.
  
• Enter and upload all pertinent information into the EBRS system.
• Once all information is uploaded successfully, DVR staff will register the birth and the State File Number will be automatically generated.
