WORK STUDY OBLIGATED SERVICE REPAYMENT TRACKING FORM 

​A participant moves into the obligated service repayment stage once they have either completed their educational studies, decided to withdraw, or if suspended from the program. During the repayment stage, a participant will repay their accrued obligated service monthly until it reaches a zero balance. Once completed, your respective Work Study Program will be closed.

The Obligated Service Repayment Tracking form must be completed and submitted to Training Services by the 15th of each month. The following forms of time are eligible for OBS repayment: regular work hours, personal leave, vacation leave and paid administrative leave. Please note that the service repayment time is calculated based on the timesheets provided. Therefore, no time is taken from your Workday account​.

Please see the following:
  1. The Obligated Service Repayment Tracking form and supporting timesheets must be submitted by the 15th of the month until a zero balance is reached.
  2. All signatures must be included on the form.
  3. All time listed on the timesheet must be in some form of approval status.
  4. If you move to another office within the Maryland Department of Health, you must inform the Training Services Division and your supervisor prior to your separation from the department.
    • A copy of the last Obligated Service Repayment Tracking form that was submitted and signed by Training Services Division staff prior to joining the new MDH office must be provided to the new supervisor and the designated human resource staff member.
    •  You must obtain the signature of your new supervisor and appointed authority moving forward.​
  5. Copies of the form reflecting each repayment must be placed and retained in the employee’s personnel file as specified in the Work Study Guidelines.
  6. You are not eligible to apply for a new Work Study Program until all obligated service has been repaid in full. 
  7. If your employment is terminated with the Maryland Department of Health for any reason, you must complete and submit the Work Study Transfer, Departure or Reduction Form to the Training Services Division prior to leaving the Department.
