PHILOSOPHY:

The Rehabilitation Service Department (RSD), is a multi-discipline service program. Services are based on the belief that everyone has the right to live a productive and meaningful existence in the least restrictive environment. This includes the elimination of barriers and development of competencies for the purpose of assisting individuals toward their optimum levels of personal, emotional, social, community, vocational and leisure development. It is our belief that greater emphasis on “wellness”, “recovery” and “responsibility of self” for his/her own actions will better prepare the individual for discharge and successful community re-entry.
  

MISSION STATEMENT / GOAL: 

The primary mission of the Rehabilitation Services Department is to provide the highest quality of patient care and treatment to the adolescent and adult psychiatric patient through all levels of care, in accordance with the contemporary professional standards and practice of the therapeutic modalities comprising the Rehabilitation Services Department.
 
The main goal is to provide discipline-specific skilled interventions for patients based on the individual needs of each patient. Treatment modalities include: the provision of experiences designed to facilitate the habilitation and rehabilitation of skills to support current strengths; address barriers; and increase motivation toward becoming self-reliant and independent.  These modalities help by facilitating the individual to routinely perform at their optimal level of functioning in keeping with their chosen goals and lifestyle.
  

SCOPE OF PATIENT CARE:

The Rehabilitation Services Department has the responsibility to assure that all patients admitted to Spring Grove Hospital Center are provided with individualized care to meet the identified mental, emotional and physical health needs. Services are provided in a safe, ethical, humane manner, regardless of diagnosis, condition, ethnic origin, age, gender or religious preference.
 
All treatment and services are provided by skilled, professional and technician level staff and are based on an initial assessment which is completed within 72 hours of hospital admission. This assessment identifies pertinent health and life history, strengths, barriers, abilities, functional limitations, needs and interests of the individual patient. Based on the assessment findings, rehab interventions are specified, in accordance with the targeted needs and treatment goals of the patient, as identified on the Individualized Treatment Plan (ITP). Services are identified to include a minimum of 2 hours of rehab intervention per day. Day, evening and weekend services are provided throughout the hospital and intervention effectiveness is monitored on an ongoing basis. Progress notes are completed either weekly or monthly in accordance with Spring Grove Hospital Center Medical Records policy and procedures.
