Who Can Obtain a Certificate?
- An individual requesting his/her own birth certificate.
- A parent named on the certificate.
- A court-appointed guardian of the individual named on the certificate. A foster care appointment is not acceptable; proof of guardianship must be submitted.
- A surviving spouse of the individual named on the certificate.
- A representative of: (1) the individual or a parent named on the certificate; or (2) the court-appointed guardian of the individual named on the certificate. The representative must have a letter signed by the individual, parent, or guardian, and certified by a notary public, stating that the representative has permission to obtain a copy of the certificate.

Birth Procedure for Obtaining Copies in Person
Birth Certificates can be obtained between
8:00am and 4:00pm
- Appear in person at the Queen Anne's County Department of Health
- Birth records are only available to the persons named on the record or an authorized representative. Authorized representatives must submit a court order or a notarized statement signed by a person named on record. If authorized to obtain a birth record, provide the full name at birth, date of birth, place of birth (County or City), mother's full maiden name, father's full name, your relationship to the person named on the record, and your name and mailing address.
- Have a birth date after 1939. Earlier birth records are not yet available from local Health Departments.
- Present photo identification
- Complete application requesting a duplicate.
- Submit payment (Cash, personal check, or money order)

Death Certificates
Procedure for Obtaining Copies
Death Certificates can be obtained between 8:00am - 12:00pm and 2:00pm - 4:00pm
- Interested person must appear in person at the Queen Anne's County Department of Health
- Complete application requesting duplicate. Click here to download the application
- Present photo identification
- Present documentation to show direct or tangible interest
- First copy is $25.00, additional copies are $20.00 each
