Adult Evaluation and Review Services

AERS is a Maryland program based in local health departments which provides assistance to frail, aged, and disabled adults. The AERS nurse conducts an on-site comprehensive evaluation to identify available community resources to help individuals remain in the least restrictive environment, with as much independence as possible.  Evaluations are conducted by a licensed registered nurse and the plan of care is devel​op by the nurse with the consult of a licensed social worker.  Interpretation and translation services are available.​

Community First Choice (CFC)

CFC is a Maryland program which provides individuals who need assistance with activities of daily living; such as bathing, dressing, meal preparation, light chores, and accessing the community. Individuals must live in the community, and meet the medical & financial criteria.


Community Personal Assistance Services (CPAS)

CPAS is a Maryland program which provides individuals who need assistance with activities of daily living, such as bathing, dressing, grooming, meal preparation, and light chores. Individuals must live in the community and meet the medical & financial criteria.​
