​MOTA (Minority Outreach Technical Assistance) MHHD's legacy program was initially born in 2001 from the Cigarette restitution program to ensure participation in the state's Tobacco and Cancer Community Health Coalition.  MOTA has grown to be a chronic disease program providing prevention, education, outreach, and linkage to care services. ​

The Enhancing Epidemiology and Laboratory Capacity for  COVID-19 Through Community-Based Strategies​ (ELC-CHW/CTCBS) grant program is a COVID-19 response program that connects state and local resources to vulnerable minority populations through trusted community voices. The Office of Minority Health and Health Disparities (MHHD) awards funding to organizations that provide outreach, education, and linkage to primary care services. In the past local health departments, community, and faith based organizations have been awarded.

Resources provided to RMPIF are designed to facilitate significant targeted investments in important economic and community development programs and promote regional and intergovernmental cooperation.

Rural Community Innovations: Transportation Planning Grant

The National Aging and Disability Transportation Center will provide funding for rural communities to develop a transportation plan that addresses accessibility and equity. Planning activities should identify transportation barriers, consider current and future community needs, actively engage older adults and people with disabilities in the process, and create or strengthen multi-sector collaborative partnerships. Selected awardees will develop a written plan of action to guide implementation of strategies supported by the community to increase the availability of accessible and equitable transportation services.

Notice of Special Interest (NOSI): Research to Address Vaccine Uptake and Implementation among Populations Experiencing Health Disparities​

This Notice of Special Interest (NOSI) highlights the need for research on strategies and interventions to increase vaccine uptake and implementation among populations experiencing health disparities in the United States (U.S.) and its territories. Research is needed to understand and address misinformation, uncertainty, distrust, and hesitancy regarding uptake of vaccines (e.g., SARS-CoV-2, pneumococcal, influenza, hepatitis B, human papilloma virus (HPV), varicella, tetanus, diphtheria, and pertussis [Tdap]) among individuals in the U.S. and its territories, especially in populations at increased risk for morbidity and mortality due to long-standing systemic health and social inequities, as well as chronic medical conditions. This NOSI is focused on individuals across the life span (i.e., all ages). The purpose of this NOSI is to solicit research that will: 1) understand and address barriers to increasing reach, access, and uptake of vaccinations among populations experiencing health disparities; 2) evaluate organizational, local, state, and federal policies and initiatives that mitigate or exacerbate disparities in vaccine access, uptake, and series completion, and 3) evaluate community-engaged interventions (e.g., expand reach, address psychosocial barriers, and social determinants of health [SDOH]) to facilitate vaccination uptake in clinical and community contexts.

​The Mental Health Service Professional Demonstration Grant Program provides funding to support and demonstrate innovative partnerships to train school-based mental health services providers for employment in high need schools and local educational agencies (LEAs).

Applicants must propose a school-based mental health partnership established for the purpose of placing graduate students of university academic programs in school-based mental health fields into schools served by the participating high-need LEAs to complete required field work, credit hours, internships, or related training as applicable for the degree, license, or credential program of each student.​

The WORC Initiative aims to fund grants that create economic mobility, address inequities for historically marginalized communities of color, rural areas, and other underserved and underrepresented communities. These grants are designed to produce high-quality employment outcomes for workers in the Appalachian, Delta, and Northern Border regions, enabling them to remain and thrive in their communities. The Initiative provides grant funds to help impacted communities develop local and regional workforce development solutions aligned with existing economic development strategies and community partnerships, promoting new, sustainable job opportunities and long-term economic vitality with a focus on equity and underserved populations. Rural Health Research Center Pro​gram

​The purpose of the Rural Health Research Center Program is to increase the amount of high quality, impartial, policy-relevant research to assist decision-makers at the federal, state, and local levels to better understand problems faced by rural communities, and provide information that will improve access to quality, affordable healthcare services.  RHRC recipients will conduct policy-oriented health services research on rural issues and synthesize the results into reports easily understood by a non-technical policy audience.  The awards provided through the RHRC Cooperative Agreement Program are designed to provide support for a four-year research center with a specific area of research concentration. The Cooperative Agreement Program will support health services research projects but excludes clinical/biomedical research and the use of funds for delivery of services.
