Parents/caregivers play a central role in child’s care and doctors and nurses rely on them to give them information about your child’s progress or your concerns. It is helpful to have all the information parents need in one place and organized to find it easily.
The entire notebook can be downloaded from the links below. It is in PDF format. You can fill the pages that you may think are important and save it on your computer. Or you can take a printout and create your own paper copy of the notebook.
