A CVS Pharmacy in Nottingham, MD informed Dr. David M. Cromwell that a patient of Dr. David M. Cromwell had a prescription for Oxycontin filled on October 1, 2010 and then tried again on October 13th to have a prescription filled for Oxycontin on one of the doctor’s prescription forms. The pharmacy called Dr. Cromwell’s office on October 13th to verify that Dr. Cromwell had written the prescription, which he had not. Dr. Cromwell believes that this person stole his prescription pad when in the office for a patient visit.

For the past year we have had problems with someone using Dr. Ogunsolas DEA number to receive medications that she did not prescribe and to patients she has never seen. We have received several fake computer generated prescriptions in her name mostly for Oxycodone 15mg and Oxycodone 30mg. The police have been contacted however we wanted you to be aware of this as well, We were told to report this to the Board of Pharmacy and that you would put out a letter to all of the pharmacies. We know for a fact that this is happening in Anne Arundel County and Baltimore City but are unaware of any where else it may have taken place at. The pads were issued to Abigail Race, PA. The event was reported to Johns Hopkins Corporate security on June 4 and is under investigation.

SINCE I WORK IN ANNE ARUNDEL MEDICAL CENTER IN ANNAPOLIS, HE ADVISED ME TO MEET WITH AN ANNE ARUNDEL POLICE OFFICER. I MET WITH OFFICER M.C. GIBSON IN AT ANNE ARUNDEL MEDICAL CENTER AND REPORTED IT TO HIM AND THAT MY PRESCRIPTION PADS WERE STOLEN. I CALLED THE DEA OFFICE LATER THAT DAY AND LEFT A MESSAGE 2 DAYS IN A ROW. IN LATER GOT A RESPONSE FROM THEM A FEW DAYS LATER AND WAS TOLD TO SPEAK WITH THE BOARD OF PHARMACY INSTEAD. I LEFT A MESSAGE FOR THEM SINCE THEY WERE AWAY UNTIL MONDAY 12/2/13 FOR THE THANKSGIVING HOLIDAY. IN THE MEAN TIME ON 11.26/13 I CALLED LOCAL PHARMACIES IN ANNE ARUNDEL AND PRINCE GEORGE'S COUNTY TO LET THEM BE AWARE OF THIS INCIDENT.
