• Submit the most recent approved Quality Assurance letter received from DDA.
  
  
• Please include proof of liability insurance with renewal application packet.
  
  
• The provider must be in good standing with the Maryland Department of Assessments and Taxation.
  
  
• The provider SHOULD NOT be on the exclusion list for the Office of the Inspector General.
  
  
• If there are changes to site location, service codes, capacity or if a site closes, the provider must submit the Addendum for a Current Licensee form.
