Emergency Plan Submission

Maryland Department of Health implemented the Hospital Discharge Assist program to assist hospitals in returning an individual to their nursing home of origin. Hospital discharge planners complete a form to request that OHCQ review a difficult placement of a patient to their nursing home of origin.
