The completed applications must be received no later than April 25, 2024 at 12 p.m. The completed application, attachments and exhibits shall be submitted in one file in PDF format, and sent to mdh.capitalbondgrant@maryland.gov with a copy to Ahmed Awad at ahmed.awad@maryland.gov.​

​​Grant Agreement

​Beginning in FY 2025, MDH wi​ll be implementing a Capital Bond Grant Agreement ("Agreement"). ​ The Agreement is a written document memorializing the terms and conditions of the Comm​unity He​alth Facilit​ies Grant Program or Federally Qualified Health Center Grant Program Awards and incorporates the terms of the Grant Award. The Agreement will be executed at the time the Notice of Award is issued. It is recommended that the Agreement be reviewed in advance so that questions can be addressed prior to circulation.

MDH Sponsored Capital Improvement Program Grants and Loans​ Overview

OFMD is responsible for the management and administration of the MDH Sponsored Capital Improvement Program Grants and Loans, which encompasses both the Community Health Facilities Grant Program and the Federally Qualified Health Centers Grant Program.​​​​​​

​Community Health Facilities Grant Program​

The Community Health Facilities Grant Program ("CHFGP") has been in existence since the 1970s and has provided millions of dollars in capital grants.  The CHFGP provides capital grants to assist in the acquisition, design, construction, renovation, and equipping of facilities to provide mental health, developmental disabilities, and drug and alcohol abuse treatment services.  It is considered essential to facilitate the movement of individuals with mental illness or developmental disabilities from State-operated facilities to the community; and to house and treat individuals recovering from substance use disorders.  The primary focus is to develop and expand supported housing units for eligible populations.

The goal is to use the CHFGP to leverage other capital and operating funds to expand community capacity, particularly to support housing for those with forensic involvement and integrate behavioral health services (i.e., mental illness and substance use disorders) that promote prevention, provide crisis and diversion services and integrate with somatic services.​

Federally Qualified Health Centers Grant Program​

The Federally Qualified Health Centers Grant Program ("FQHCGP") has been in existence since 2004 and has provided millions of dollars in capital grants.  The FQHCGP provides capital grants to assist in the conversion, acquisition, design, construction, renovation, and equipping of Federally Qualified Health Center facilities, which provide primary care, specialty care, OB/GYN, dental, mental health, and drug and alcohol abuse treatment services.  Priorities for funding are set by the Primary Care Office under the Office of Population Health Improvement.​
