Maryland Medicaid ​Sanctioned Providers

Maryland Medicaid suspends or excludes some providers from working with the program. See the sanctioned providers list for full and current details. The program updates the list monthly if needed.

For sanction type LB on the Sanctioned Providers list, verify the provider’s license status through their Maryland State Board.

To verify a sanctioned provider, email mdh.providerexclusions@maryland.gov. Include all available identifying information such as NPI, provider license type and license number.

An exclusion record identifies parties excluded from receiving Federal contracts, certain subcontracts, and certain types of Federal financial and nonfinancial assistance and benefits. Exclusions are also referred to as suspensions and debarments.
