Medicaid covers the cost of long-term care in nursing facilities for low income individuals who meet Medicaid eligibility, medical, and financial requirements.

You can apply for long term care in the following ways:

• Online through MDThink ​​​​​​​​​​​​
  
• In person or by mail. You can go in person to or mail your completed long term care application and required documentation to:
  • ​​If you live in Baltimore City or Baltimore, Anne Arundel, or Prince George’s counties, the Bureau of Long Term Care, 346 Frederick Rd., Catonsville, MD 21228
  • If you live in other counties, your local department of social services.
