Assistance in Community Integration Services (ACIS) is a Medicaid program that may help you if you are experiencing homelessness or at risk of becoming homeless. Through the program, you will get housing related case management services and supports. The goal is to help you live safely and independently in your home and community.
You will have an ACIS case manager who will work with you to make a supportive housing plan. Then they will help find the services you need based on your plan.
Eligibility
To be eligible for ACIS, you must:
- Have Medicaid in Maryland.
- Meet one or more of these health criteria:
- Have two or more chronic conditions – that is long term illness.
- Use the emergency room or be in the hospital more than four times a year.
- Meet one of these housing criteria:
- Experiencing homelessness or at risk of homelessness.
- Facing institutional placement.
- Choose to be in the ACIS program.
You must also live in a current service area:
- Baltimore City
- Cecil County
- Montgomery County
- Prince George’s County
Benefits
Your housing case manager may:
- Do community integration assessments to identify your needs and preferences.
- Create a person-centered community integration plan.
- Take part in meetings about housing support, crisis management, and person-centered planning.
- Help connect you with social services from your community integration plan.
- Talk to the property manager or landlord about any need for accommodations.
- Connect you with training and resource support to help you manage your home.
The ACIS program does not cover room and board or rental costs.
