• Application Guidance on CCN

  • The application requires practices to provide a CMS Certification Number (CCN), also known as a 'Medicare ID Number'. Practices should first try to find their CCN by using this guidance​. If you experience technical difficulties or do not have a CCN, please email MarylandModel@cms.hhs.gov.
