CTO Comparison Tool

Welcome to the Care Transformation Organization (CTO) Comparison Tool for the 2024 Program Year. 

​CTOs are a key part of the Maryland Primary Care Program (MDPCP) and provide care management resources, infrastructure, and technical assistance to practices that choose to partner with a CTO. This website allows practices applying to participate in the MDPCP to view CTOs currently available in their area. Additional CTOs that have been newly approved to participate in MDPCP will be added later this summer, prior to the CTO pairi​ng window in the fall.  

New applicants interested in partnering with a CTO will have the opportunity to write in their first and second choice of CTO partner as part of the application. Both currently participating practices and new applicants will have the opportunity identify a first and second choice of CTO partner during the CTO pairing window in late summer/early fall of each year. 

P​ractices are not required to partner with a CTO in order to participate in the MDPCP. For more detail about partnerships between CTOs and practices, please see the Request f​or Appli​cations (RFA).

  • To view a list of CTOs that are available in your county, please select the county in which your practice is located. 

  • To learn more about the services provided by a particular CTO, please visit their website.

  • ​To view a comparison of CTO performance on various metrics, please click her​e​​. 

  • New applicants interested in partnering with a CTO will have the opportunity to write in their first and second choice of CTO partner as part of the application.​ This choice will then be confirmed via an online form that will be emailed to practices directly in the Fall. 

  • Current program participants will have the opportunity to select a new CTO for the 2024 Program Year during an open enrollment period that will take place in Fall 2023​.  A link to an online CTO Selection Form will be emailed to practices directly.
