Grant and Amount: 20-030 ($90,000) 
Grant Project Description: Upper Shore Aging, Inc. will increase diabetes risk screening for all low-income seniors served by Upper Shore Aging, including those attending senior centers and home-bound seniors. Home screenings will be performed in partnership with Meals on Wheels. The program seeks to increase awareness of diabetes risk factors and provide risk prevention education and increase food security by improving access to healthy fruits and vegetables delivered with Meals on Wheels home meals. The program will seek increased collaboration among health care providers and will work with the Kent County Health Department and Kent County Department of Social Services to increase no cost access to Diabetes Prevention Programs for Medicare recipients.

Focus Area: Childhood Obesity
Grant and Amount: LHIC 12-001 ($25,000) 
Grant Project Description: This one year grant supported Kent County Health Department’s efforts to  increase their capacity to address DHMH’s  State Health Improvement Process (SHIP)​ goals. SHIP focuses on improving population health outcomes and measures every jurisdiction on its performance on 39 population health metrics.

Grantee Organization: Choptank Community Health System 
Focus Area: Dental
Grant Amount: 11-004 ($130,000) 
Grant Project Description: The two-year grant supported a partnership between CCHS and the Chester River Hospital Center to provide pediatric dental surgery services in Kent County, a Medically and Dentally Underserved Area (MUA). Grant funds were utilized to purchase equipment for the operating room and provided the “bridge” funding for the reimbursement differences between anesthesia costs and Medical Assistance reimbursement. This program provided care to 163 underserved children on the Eastern Shore with complex dental needs.
