Grant Project Description: This two year grant supported efforts to increase access to health care and enhance chronic disease prevention by utilizing “hotspotting” data analysis and targeting resources to address complex health needs. Grant funds were utilized to establish a community health worker program; increase the number of PCMHs in the community; and create a shared savings blueprint.

Grant and Amount: LHIC 12-013 ($25,000) 
Grant Project Description: The Howard County Local Health Improvement Coalition utilized grant funding to support efforts to strengthen the coordination and collaboration among community providers and other resources to more effectively address access to health care and behavioral health services and to encourage people to achieve and maintain a healthy weight.  Grant funds supported the implementation of three projects: (1) a Biennial Health Survey, jointly sponsored with the Horizon Foundation, to gather baseline ethnic health information not previously collected; (2) the development of “Sugar Sweetened Beverage Campaign,” a marketing campaign aimed at reducing sugar sweetened beverage consumption, jointly sponsored by the Horizon Foundation; and (3) a Behavioral Health Inventory to improve understanding of the referral and release processes of three behavioral health major providers.

Grant Project Description: This two-year grant supported the Howard County Health Department, in partnership with the Anne Arundel County Health Department and DHMH, to pilot an electronic application system that screened and enrolled families into multiple public assistance programs with one application. The online program provided screening and enrollment for individuals and families in programs such as Medicaid, Maryland Children's Health Program, and the Primary Adult Care Program.

Grantee Organization: Chase Brexton Health Services, Inc. 
Grant and Amount: 09-013 ($500,000) 
Focus Area: Dental 
Grant Project Description: This two-year grant supported Chase Brexton in expanding services at the Columbia Center dental clinic for adult and pediatric patients. Grant funds were utilized to provide students with oral hygiene education, oral health screenings, fluoride varnish, sealants to molars, and referrals for dental services at Chase Brexton. Over the duration of the grant, the dental clinic served 4,708 patients through 5,716 visits.

Grant and Amount: 07-005 ($200,000)

Grant Project Description: This two-year grant supported a program aimed at reducing avoidable visits to the emergency department at Howard County General Hospital. The program employed outreach workers in the ED to connect patients to a Chase Brexton health center location for follow-up care. Funds were used to support personnel costs and to implement a digital interface between the hospital and Chase Brexton. The program served over 600 patients and provided for over 900 patient visits in ambulatory care settings.
