Grant Project Description:
 This program will support the HOMEBUILDERS model, which is an evidence-based family preservation program that serves impacted by the opioid crisis who are referred by Child Protective Services, with specific focus on infants and children at serious risk for removal from the home. The program provides intensive in-home services to vulnerable families with complex health and social service needs over a 28-day period, and referrals for specialized addiction services outside the home.

Grant and Amount: 19-012 ($275,000) 
Grant Project Description: This project would expand access to screening and substance use treatment services in schools throughout Baltimore County.  The project would develop and implement a systematic prevention, early detection, and referral program to prevent substance use.  CHRC grant funds would support a contract with the Center for School Mental Health at University of Maryland School of Medicine and mini-grants to substance use treatment agencies selected via RFP to provide Certified Addiction Counselors at participating schools at least one day per week.  The program is designed to be implemented in all high schools, and other support services staff (school counselors and social workers) would be trained on the use of the CRAFFT 2 screening tool and the brief negotiated interview.  The project would also establish community partnerships between each high school and alternative programs in BCPS.

Grant Project Description: This project award supports the TOGETHER Project which provides counseling and continuing care services to children and adults affected by the opioid crisis. The project seeks to keep families intact and encourage individual self-efficacy and resilience. In addition to specialized group therapy, participants receive wraparound social services through the partnership with Catonsville Emergency Assistance to supply links and resources to clients facing eviction, foreclosure, utility cut-offs, and food insecurity. The project will increase knowledge about addiction among the affected family members and awareness of the Opioid Crisis in the general public.​

Grant and Amount: LHIC 12-005 ($25,000) 
Grant Project Description: The Baltimore County Health Coalition utilized CHRC grant funding to support efforts to address childhood obesity and enhance the Healthier Generation Healthy School Program, which includes 25 public schools.  CHRC grant funding supports staff professional development and data analysis on Body Mass Index (BMI).

Grant and Amount: 11-008 ($350,000)
Grant Project Description: This three-year grant supported a re-entry program that provided case management services for incarcerated patients before and after release with behavioral health and substance abuse disorders. The program directed participants with substance abuse disorders to available inpatient and outpatient substance abuse programs post discharge from a detention center.  Grant funds supported two case managers who were geographically distributed throughout the county. This program provided services for 344 individuals over the duration of the grant.

Grant and Amount: 10-002 ($293,759)
Grant Project Description: CHRC grant funds supported the Prenatal Enrichment Program, which provided access to a prenatal, educational and case management services for high risk pregnant women and teenagers. The program provides education and counseling on management of health issues, screenings, and interventions for smoking and other substance use during pregnancy, and home visiting program during pregnancy and after delivery. Grant funds were utilized to support the salaries of program staff. The grantee reported serving 2,019 women through 4,275 visits over the duration of the grant.

Grant and Amount: 09-003 ($55,450) 
Grant Project Description: This one-year grant supported the health department’s efforts to enhance the information technology capacities in the health department's school-based wellness centers, and to expand services by opening a school-based wellness center in Dundalk. Grant funds were utilized to upgrade the computers systems at the SBHCs. The new computers and other upgraded equipment paid for with this funding enabled the SBHC to keep more complete and accurate patient records with an ultimate goal of being able to bill for health care services rendered.
