Community Health Resources Commission Issues Annual Request for Applications (RFA); $7 million to be provided in new grant funding.  

Under this funding opportunity, the CHRC will make a total of $7 million available to support projects in the following four areas: 

  1. ​Addressing chronic disease prevention and disease management, including diabetes and its comorbidities, hypertension, heart diease and others; 
  2. Promoting maternal and child health 
  3. Providing dental care to support the Maryland Medicaid Dental Benefit 
  4. Addressing Behavioral Health, including Mental Health and Substance Use Disorder (SUD) and the ongoing impact of the opioid crisis.​​
​​The RFA is open to public health agencies, federally qualified health centers, saftey-net providers, free clinics, community-based organizations and behavioral health providers.  This RFA strategically prioritizes programs that address health disaparities and adverse Social Determinants of Health (SDOH) and promote the delivery of integrated health and social services in underserved communities.   Applicants can request up to 3 years of funding.  The CHRC has the discretion to make awards in any amount; and anticipates making awards in the amount of $300,000 - $750,000 total per grant.
