Health Equity Resource Communities

The Maryland Community Health Resources Commission received a total of 59 eligible applications requesting $291 million in funding.  Click here​ for a list of applicants by jurisdiction.  The applications are currently under review and applicants will be notified of their application status in late March 2024. 

 The Health Equity Resource Communities (HERC) grant program will provide $42 million in new grant funding​​ to reduce health disparities and improve health outcomes in underserved communities.  The new funding builds on the prior Pathways to Health Equity grant program and is made available under the Maryland Health Equity Resource Act approved by the Maryland General Assembly in 2021.

“The Maryland Health Equity Resource Act,” approved during the 2021 legislative session, provides significant new grant funding and state resources for local communities to address health disparities, improve health outcomes, expand access to primary care and prevention services, and help reduce health care costs. The Maryland Community Health Resources Commission (CHRC) will implement this Act with the assistance of the Health Equity Resource Community (HERC) Advisory Committee, the Maryland Department of Health’s Office of Minority Health and Health Disparities, and Chesapeake Regional Information System for our Patients (CRISP).
