Chronic Unit

Deer's Head Hospital Center's Chronic Unit is a Specialty Hospital, specializing in medically complex patient care. Our goal is to provide comprehensive care to all our patients by providing specialized service to enhance the patient outcomes and return to their home setting.

Our state of the art facility provides weekly interdisciplinary care planning for all of our patients in order to enhance their stay and their outcome. Our facility provides discharge planning to our patients to provide them with all of their needs. We have a new transitional apartment available for our patients to prepare them for their discharge home.

We accept patients with multiple diagnosis and conditions; traumatic brain injury, new amputees, cerebral vascular accidents, traumas, all of which our goal is to give them the tools they need to return to a home setting with the best outcomes.
