What is a CCS?
DDA's Coordination of Community Services (CCS) providers help individuals with intellectual or developmental disabilities and their families by connecting them to community resources, planning for the future, and accessing necessary services. In Maryland, CCS providers, also known as Targeted Case Management, ensure that individuals and families receive the needed support, training, and services for self-sufficiency, health, safety, and active community participation, while following federal and state guidelines.
What does a CCS do?
CCS providers assist individuals with intellectual/developmental disabilities and their families in learning and gaining access to resources in their community, planning for their future, and accessing needed services and supports.

In Maryland, CCS providers fulfill the role of "Targeted Case Management." Medicaid Targeted Case Management (TCM) is a service that manages multiple resources for Medicaid members. It is designed to help persons with intellectual disabilities, brain injury, or developmental disabilities gain access to appropriate and necessary medical services and interrelated social and education services. To be a CCS provider in Maryland, a provider must meet training, staffing qualification, and other state and federal requirements for case management/care coordination services, and meet federal criteria for conflict-free case management.
