Health Planning

The Carroll County Health Department creates and maintains a variety of plans to ensure our services are meeting community needs and reducing gaps and barriers to our vision of a safe and healthy community for all. Our planning processes involve staff at all levels of the agency, representative of all four bureaus. We review local, state, and national data, work with community partners, ask for feedback from customers, and study best practices to create plans that help us address public health priorities in Carroll County. 

The Health Department’s Health Planning efforts include agency-wide strategic planning, quality improvement, and performance management. We use health data from a wide variety of sources, and use it to identify health trends and plan initiatives. We create a local public health plan for Carroll County using community needs assessment data and State Health Improvement measurements specific to Carroll County. We work closely with many community partners to help assess, prioritize, and address community health needs.

The Health Department works closely with The Partnership for a Healthier Carroll County, which leads Carroll County’s Community Health Needs Assessment. This extensive assessment includes individual community surveys, focus groups with target populations, and key informant interviews, as well as secondary data. This data is used to inform Carroll Hospital’s Community Benefit Plan and Carroll County’s Carroll County Local Health Improvement Plan 2022
