Reactivation / Reinstatement Online Application

BEFORE YOU BEGIN, HERE ARE SOME HELPFUL TIPS:
  1. This is a two step process:  a. The application and payment are completed online. b. Your continuing education report form and certificates are submitted per instructions below. ​​​​​
  2. You must have an active email address
  3. You cannot complete applications with your phone or tablet. You must use a desktop or laptop computer.​
  4. To determine whether to apply for Reactivation or Reinstatement:
    If your license is currently “Inactive” (I), apply for Reactivation. 
    If your license is “Not Renewed” (N), apply for Reinstatement. 
    If you do not know, check your license here
  5. If your license has been non-renewed or inactive for more than 5 years you will need to apply for Reissuance. Please email Marianne Graham, Director of Licensing and Certification at [email protected] for more information.
The Reinstatement/Reactivation Process: ​​

Step 1:  You must Apply and pay the application fee. Link to ONLINE APPLICATIONS​

Step 2: You Com​plete the Continuing Education Report Form ​​​​​​

Step 3  You Submit the signed continuing education report form and the continuing education certificates to Stacie Rigby, Continuing Education Associate at [email protected]. ​​

Step 4: You must Complete a Criminal History Records Check​​

Step 5: You must C​​​omplete and submit the FBI Privacy Statement per the instructions.​ Click HERE to complete
