​Things to Know Before Submitting an Application

  • The Board has five types of applications for acupuncturists: Initial, Renewal, Reinstatement Inactive, and Reactivation. You can also find below applications relevant to Military & Veterans, Change of Name/Contact Information, Auricular Detoxification, and Animal Acupuncture. To access an application, please scroll down this page and click on the appropriate tab. All applications can only be completed online. 
  • Payment for all applications must be made by credit or debit card via the Board's secure online portal Authorize.net. For the applications, you will be automatically directed to the Authorize.net payment portal or be provided a link to the payment portal. Please contact the Board if you have difficulty gaining access.
  • Applications are not submitted until payment is made. An applicant should be sure that he/she should be able meet the requirements of the application process before payment is made. The Board does not refund application fees.
  • A valid email address is required to use the online system. Application identification numbers and status updates are directed to the applicant's email address. To ensure ease of communication, please add the Board's email address -- mdh.acupuncture@maryland.gov -- to your trusted/allowed email address list and update your spam folder filter to ensure communication from the Board's email address is directed to your inbox.
  • Supporting documentation can be submitted by email, fax, mail, or file upload through Google Forms.
  • Please contact the Board, before starting the application process, if you do not have a Social Security Number (SSN) or an Individual Tax Identification Number (ITIN).
  • Please contact the Board if you require a reasonable accommodation to complete an application process.
  • The amount of time it takes to complete the application will vary depending on an applicant's response and the amount of detail he/she includes.
  • Once submitted, the amount of time required for the Board to approve or deny an application varies based on when the applicant submits all required documentation to support the application.
