Information

To become a b​ody donor please download the ‘D​onor Packet’ at the link below.  Review the information carefully and complete the form as per the instructions. If you have any questions or need clarification, please contact the State Anatomy Board and we’ll be happy to answer any of your questions. ​​​

Completed Donor Forms with original signatures​ will only be accepted by mailing to address in instructions .  DO NOT EMAIL.

​Please be advised that due to backlog of forms, registration may take up to 60 days to be accepted and processed.​​​​​​​

Updating Donor Informaiton

If you are currently a body donor and would like to update your current information, please use the following update form listed below to make changes as needed. The following form should only be mailed to the SAB if you are currently a donor. New donors must use the form provided above. ​
