The PATH Project is an outreach program for Allegany County residents with a mental illness who are homeless or at risk of becoming homeless. The program provides a variety of services including outreach/case management, advocacy, and assistance in applying for housing and Supplemental Security Income (SSI). The program may also provide assistance in accessing healthcare, behavioral health services, job opportunities, and educational resources.
PATH staff works very closely with the New Horizon Re-Entry Program, the inpatient psychiatric unit at the Western Maryland Regional Medical Center, Union Rescue Mission, Family Crisis Resource Center, Laura’s Anchor Transitional Housing Shelter, and others to identify and assist individuals who meet eligibility requirements for this program.

Continuum of Care (CoC) Housing Program:
This program provides tenant-based rental assistance to homeless individuals and families with an adult member who has a serious mental illness or co-occurring substance use disorder. Individuals and families are provided with supportive services through the Public Behavioral Health System and other state or local agencies.
