File a Complaint
If you have concerns about your health care or treatment, you have the right to make a formal complaint.
The Office of Health Care Quality offers three options to file a complaint. Your complaint will be investigated in accordance with Maryland statute regulations. You may make an anonymous complaint; but we won’t be able to send you a written report of the investigation.
- File a complaint on the OHCQ website through the Online Complaint Report Form. The online complaint form will allow you to upload any supporting documentation
- Complete and mail in a Paper Complaint Report Form. Please submit documentation supporting your complaint with the complaint form when you return it to the Office of Health Care Quality either by mail or fax.
- Complaints over the telephone can be made by calling the appropriate Unit:
