Health Consumer Complaints
If you have concerns about your health care or treatment, you have the right to make a formal complaint with the Office of Health Care Quality. If this is a true emergency dial 911.
File a Complaint
If you have concerns about your health care or treatment, you have the right to make a formal complaint.
The Office of Health Care Quality offers three options to file a complaint. Your complaint will be investigated in accordance with Maryland statute regulations. You may make an anonymous complaint; but we won’t be able to send you a written report of the investigation.
File a complaint on the OHCQ website through the
Online Complaint Report Form
. The online complaint form will allow you to upload any supporting documentation
Complete and mail in a
Paper Complaint Report Form
. Please submit documentation supporting your complaint with the complaint form when you return it to the Office of Health Care Quality either by mail or fax.

Developmental Disabilities Programs
Health Maintenance Organizations
