Steps Individuals Must Take to Apply for Services
An individual (or other interested party), may either walk in to the local Health Department OR call in advance to schedule an appointment with a coordinator to discuss service requests.
To apply for Developmental Disabilities Administration (DDA) funded services, an individual will need to complete a DDA Application for Services form, and a Critical Needs Recommendation form. They will also need to submit with their application some documentation of their disability. These forms may be obtained on-line or in person from any Resource Coordinator.
