  • Board Letter - Tax Delinquency & Child Support Notification (via phone, email & USPS mail)
  • Board Letter - Failure to Notify of a Change of Address ( posted & mailed via USPS)

 

What do I Need Before I Begin to Renew My Registration Online?

  • A Computer, Laptop, or Notebook (not a smartphone)
  • My  License Number from either the following:
  • Completed CEUs and their respective course completion certificates (copies) from Sept. 2, 2017 through Sept. 1, 2019
  • Copy of my current CPR certification (Healthcare Provider Level)
  • Credit Card
  • Printer to print out a copy of my completed Application AND Receipt for my records; or
    • in case I am Audited to which I have complete the application process and paid
    • print out the application and receipt (x2)
    • Email (mdh.chiropractic@maryland.gov) or Fax (410 358 1879) or Mail directly to the Board a packet containing: (1) copy of the application, (2) a copy of the receipt, (3) copies of CEU course certificates, and (4) a copy of CPR card.
