Initial License

How long is a license valid?  Effective January, 2016, all occupational therapist and occupational therapy assistant licenses issued will be a two-year license.  

What are the requirements?  A completed application form, $200 application fee, two moral character forms (until January 31, 2018), a completed State and FBI Criminal History Records Check​ (beginning February 1, 2018), verification from any state or country in which you have been licensed, successful completion of the Board's jurisprudence examination, verification of initial/current certification from NBCOT, and continuing competency requirement hours.  CCR requirement is waived if NBCOT certification is current at time of application.

Can I send a scanned copy of my check or money order?  No, you must mail in your $200 payment.

When can I expect my license?  Routine applications are reviewed internally and then ratified by the Board at its monthly Board meeting on the third Friday of every month.  They are reviewed for approval as soon as the application process is completed.

What does not qualify as a routine applications?  These include character/fitness/disciplinary history issues, no preapproved continued competency requirement hours, and/or a lapse in practice of three or more years.  Special case applications are reviewed by the Applications Review Committee on a case by case basis.

Do I have additional requirements if I have been out of practice for 3 or more years?  Yes, the additional CCR requirements can be found here.

Will I receive a paper license?  No.  The Board went green in 2013 and no longer prints paper licenses.  Licensure status may be verified in real time via the Board's verification page.
